The Benefits of Medication-at-Home

Taking medication is not without risks, especially for the elderly. Drug interactions can have unexpected and unintended effects. Side effects can lead to complications. Directions can be misunderstood. The recommended dosage can require adjustments. The list goes on and on.

Traditionally, doctors and other medical professionals had close interaction with the elderly patients whom they treated in assisted living or rehabilitation facilities. They were readily available for fielding questions and could quickly address issues related to prescription medications.

Today, however, medical models are changing. Many elderly patients are turning to the “aging in place” concept of medical care. This model empowers those with chronic medical conditions to receive appropriate care in their homes rather than at an assisted living or rehabilitation facility. 

The aging-in-place model offers a host of benefits, including better affordability and the ability for patients to stay more connected to their social circles. However, it also creates a distance between patients and their doctors that is much greater than they experience in care facilities. Because of this distance, patients have less available guidance from doctors when it comes to managing their medication.

Supporting aging-in-place with medication-at-home

To fill the gap created by the aging-in-place model, the world of healthcare is turning to an innovative method of medication management that is referred to as “medication-at-home.” This new model for patient care looks to pharmacists to provide an enhanced level of care for their patients. It looks to the clinical expertise that pharmacists bring to the care continuum as a key resource for patients who are looking to use medication safely and effectively while aging-in-place.

Appreciating the important role that pharmacists play in this new health landscape requires understanding how often pharmaceuticals are prescribed to the elderly. In 2010, studies showed that 87 percent of US adults 65 years-of-age or older used at least one prescription medication. By 2019, that number had grown to 89 percent. Elderly patients who do not use prescription drugs have come to be the rare exception. 

It should also be noted that aging in place may soon be a necessity rather than an option. The aging population in the US is leading to what some are calling an assisted living crisis. Those in the 65 and older category comprise approximately 46 million Americans today. By 2050, that demographic is expected to nearly double. As the number in need of care continues to increase, finding space in assisted living facilities may prove to be an overwhelming challenge, leaving home care as the only available option.

Providing expert medication oversight

For those experiencing aging-in-place, the medication oversight that the medication-at-home model provides is essential. Polypharmacy, which is the term used to describe the use of multiple medications, is a reality for most elderly receiving medical care. Figures on polypharmacy show that among those 65 or older, an estimated 44 percent of men and 57 percent of women take five or more medications.

Pharmacists have the expertise that is necessary to provide medication reconciliation for those who are taking multiple prescription medications. This process involves pharmacists reviewing the entire listing of medications that a patient is taking to determine overlaps or possible adverse reactions that may result from combinations of drugs.

In an assisted living setting, doctors would have access to the full slate of medications that patients were using. With aging-in-place, patients may receive prescriptions from a variety of doctors, including their primary care physician and other specialists. Pharmacists may be the only ones who have the complete picture of the medications that aging-in-place patients are taking.

Pharmacists are also uniquely positioned to provide essential guidance to patients as they begin with new medications, such as those that might be prescribed after a hospital visit. Studies show that 20 percent of hospital readmissions are medication-related. Among those, 70 percent were considered preventable. When pharmacists step in to assess how new medications might interact with what a patient is already taking, as well as to provide clarity on the proper procedure for taking new medications, unnecessary hospitalizations can be avoided.

Medication reconciliation is an ongoing part of the medication-at-home process. It begins during the onboarding process and is assessed on an ongoing basis. Rather than simply filling and billing prescriptions without oversight, medication-at-home elevates the process to the level of clinical pharmacy care. As a result, aging-in-place patients receive the same attention to detail that they would receive from caregivers at an assisted living or rehabilitation facility.

Supporting healthy adherence

To experience the full benefits of their prescriptions, patients must adhere to the prescription guidelines. If they fail to take the right dose at the right time under the right conditions, the medical benefits of the medication can fail to be realized. Lack of adherence is thought to be one of the biggest risks for patients who are taking medications in an aging-in-place setting.

The medication-at-home model can provide significant adherence support to patients. One solution it provides is medications that are synchronized to one fill day and cycled into a continual fill date. This takes the confusion that can result from refills off of patients and the caregivers who support them. By packaging medications in date and time stamped pouches, patients have a much easier time of adhering to their medication schedules.

In addition, the medication-at-home process also facilitates the same type of multi-dose medication packaging that is made available at onsite care facilities. This method, which is proven to increase success rates with medication adherence, places all of the medications that must be taken at the same time in the same packaging, removing the need for patients to institute their own organization methods.

To further support the medication process, pharmacists can coordinate delivery services that ensure patients have what they need when they need it. Throughout the entire process, the medication-at-home team stays in contact with the patient to provide coaching, counseling, and whatever other reassurances are needed.

Reducing healthcare costs

Finally, medication-at-home facilitates a solution that makes effective healthcare much more affordable for elderly patients. By 2028, out-of-pocket costs for facility care are expected to total more than $266 billion. Aging-in-place dramatically reduces those costs. By leaning on the historically underutilized expertise that pharmacists bring to the healthcare puzzle, medication-at-home makes aging-in-place and the benefits it brings to patients a viable healthcare solution.

Extend the PSLF Waiver

Menendez, Kaine, Gillibrand, Van Hollen, Sarbanes, Hayes, Courtney Urge the Biden Administration to Extend the PSLF Waiver.

The bicameral group of lawmakers asked for the waiver deadline to be extended until July 1, 2023

U.S. Senators Bob Menendez (D-N.J.), Tim Kaine (D-Va.), Kirsten Gillibrand (D-N.Y.), and Chris Van Hollen (D-Md.), alongside U.S. Representatives John Sarbanes (D-Md.-03), Jahana Hayes (D-Conn.-05), and Joe Courtney (D-Conn.-02), today led a bicameral group of nearly 100 colleagues in urging U.S. Secretary of Education Miguel Cardona to extend the limited Public Service Loan Forgiveness (PSLF) waiver deadline until July 1, 2023 – when the Department of Education’s new PSLF regulations are set to take effect, in order to ensure all public servants with federal student loans can benefit from this historic waiver. Since the Department announced the limited PSLF waiver last October, more than 189,000 public servants have had their student loans forgiven through the program and one million have received an average of one additional year of PSLF credit.

“To date, the waiver has been overwhelmingly successful in reducing barriers for borrowers to receive PSLF relief, as the waiver accounts for almost all (91%) of the borrowers who have received forgiveness through the PSLF program through July 31, 2022,” wrote the lawmakers to Secretary Cardona. “The limited waiver [has] also [been] a lifeline for Federal Family Education Loan (FFEL) borrowers who—for the first time—have seen their payments acknowledged in the PSLF program.”

Despite how many public servants have already benefited from the limited waiver, the lawmakers highlighted that these borrowers represent only a fraction of the public servants who are eligible for PSLF. According to the Student Borrower Protection Center’s estimates, only 15% of the 9 million public service workers with student debt have filed paperwork to track their qualifying payments under PSLF. The lawmakers also echoed the more than 20 state attorneys general who warned that fundamental programmatic problems with the PSLF program will immediately return – likely in a more exacerbated manner – if the waiver ends before the Department of Education implements its new PSLF regulations.

“Extending the PSLF waiver will also allow more military service members and federal employees to make progress towards loan forgiveness through PSLF. Though the Department has announced that it is currently working to ensure that all federal agencies and departments automatically provide credit for PSLF for members of the military and federal employees, this program is not yet in place,” added the lawmakers. “Military and federal data matches are critical to ensuring that the Department can identify which borrowers are eligible for PSLF and provide them automatic relief.”

Sen. Menendez and many of his colleagues have long called on the Department of Education to make comprehensive fixes to the PSLF program, which has faced scrutiny for several implementation challenges since its inception in 2007, with only 1 to 2 percent of PSLF applicants being approved each year prior to the Biden Administration proposing a sweeping overhaul of the PSLF program. Today’s effort comes more than a month after Sen. Menendez and several of his colleagues led a group of more than 100 lawmakers in calling on the Biden Administration to extend the PSLF waiver deadline.

Dear Secretary Cardona:

Thank you for your continued efforts to improve the Public Service Loan Forgiveness (PSLF) program. As you are aware, the limited PSLF waiver, announced last October, has significantly improved the lives of the more than 189,0001 public servants who have had their student loans forgiven though the program and the one million2 who have received an average of one additional year of PSLF credit. Given the upcoming October 31, 2022 limited PSLF waiver deadline, we write to reiterate our request3 that the Department of Education extend its deadline until July 1, 2023—when the Department’s new PSLF regulations will take effect—in order to ensure that all public servants with federal student loans are able to benefit from this historic waiver. 

To date, the waiver has been overwhelmingly successful in reducing barriers for borrowers to receive PSLF relief, as the waiver accounts for almost all (91%) of the borrowers who have received forgiveness through the PSLF program through July 31, 2022.4 The limited waiver is also a lifeline for Federal Family Education Loan (FFEL) borrowers who—for the first time—have seen their payments acknowledged in the PSLF program. As more than 20 state attorneys general have pointed out, “[g]iven the essential benefits provided by the limited PSLF waiver, and the fact that fundamental problems with the PSLF program will immediately return (likely in an exacerbated form) upon the waiver’s end, we have grave concerns about the plans to end the waiver . . . before the Department’s new PSLF regulations take effect.”These attorneys general, alongside the education advocacy, government, and non-profit community as a whole, agree that operationalizing and explaining the waiver has been an incredible challenge for the Department, as well as its servicers. Such challenges have left numerous public servants confused about how the waiver, consolidation, and the Department’s one-time review to fix forbearance and deferment failures will work. This is especially true for FFEL borrowers and borrowers who still hold joint consolidated loans from the Department, many of whom are still unaware that they may have an opportunity to qualify for PSLF under the limited PSLF waiver.

Importantly, extending the PSLF waiver will also allow more military service members and federal employees to make progress towards loan forgiveness through PSLF. Though the Department has announced that it is currently working to ensure that all federal agencies and departments automatically provide credit for PSLF for members of the military and federal employees, this program is not yet in place. Military and federal data matches are critical to ensuring that the Department can identify which borrowers are eligible for PSLF and provide them automatic relief.

Since the deadline of October 31, 2022 to qualify for PSLF under the waiver program is less than one month away, we ask that the Department extend this deadline in order to ensure that all public servants with federal student loans are able to benefit from this historic waiver.

The SUV for the Electric Age – Polestar 3 Premieres on October 12

Polestar cars (Nasdaq: PSNY) will premiere its first SUV, Polestar 3, at a launch event on October 12, 2022, in Copenhagen, Denmark. Featuring Polestar CEO, Thomas Ingenlath, and Polestar Head of Design, Maximilian Missoni, the event will be live-streamed globally beginning at 1 p.m. EST, allowing consumers a glimpse at the brand’s debut in the SUV market. Polestar 3 is a powerful, design-led electric performance SUV that appeals to the senses with a distinct shape while providing excellent driving dynamics. “We had to ask ourselves: ‘what should a modern SUV look like?’ Polestar 3 defines the SUV for the electric age, and in designing it we addressed some of the fundamentals of the archetype: proportions, stance, and aerodynamics,” says Thomas Ingenlath, Polestar CEO.

Built on a new all-electric technology base, Polestar 3 also features the latest in high-tech components. This includes a centralized core processor from NVIDIA, and a long list of advanced safety systems from Volvo Cars and leading industry suppliers such as Zenseact, Luminar and Smart Eye.

At Polestar, performance means more than straight-line acceleration. At launch, each version of the Polestar 3 will feature a rear-biased dual motor powertrain and torque vectoring via a dual-clutch system on the rear electric motor. Delivered as standard, adaptive dual-chamber air suspension, and active dampers will allow Polestar 3 to switch between comfort and firm suspension dynamics while adjusting to the road conditions once every two milliseconds (500 Hz).

“It’s this instant ability to transform from a comfortable cruiser to a sharp, agile performance car in less than the blink of an eye that makes Polestar 3 special as an electric performance SUV,” continued Ingenlath. “It also benefits from a low center of gravity and wide track for ultimate stability, and an exciting feeling behind the wheel.”

The optional Performance Pack provides the Polestar 3 with 517 hp and 671 lb.-ft. of torque, along with Polestar Engineered chassis tuning for the active dampers and air suspension. Polestar’s signature Swedish gold details are also added, including valve caps, seat belts and a laser-etched interior light strip.

Polestar has produced two electric performance cars. The Polestar 1 was built between 2019 and 2021 as a low-volume electric performance hybrid GT with a carbon fiber body, 619 hp, 738 lb-ft of torque, and an electric-only range of 52 miles (EPA) – the longest of any hybrid car in the world.  

The Polestar 2 electric performance fastback is the company’s first fully electric, high-volume car. The Polestar 2 model range includes three variants with a combination of long- and standard-range batteries as large as 78 kWh, and dual- and single-motor powertrains with as much as 476 hp and 502 lb-ft of torque.

The hard-top convertible presents an evolution of the unique design language first shown by Precept and emphasizes a dynamic driving experience. The concept further develops the focus on sustainability and technology, aiming towards greater circularity. 6 Common Misconceptions about Probiotics

Many people tend to have mixed reactions to probiotics because of a few myths and misconceptions surrounding them.

Today the market is flooded with an endless variety of supplements which can create confusion in consumers.

Probiotics are available in almost every pharmacy and most grocery stores since they have become a popular addition to people’s diets.

However, they are often advertised incorrectly which can result in people receiving incorrect information about them.

1. I Get Enough Probiotics From My Diet

Most people tend to think that certain food items in their diet like yogurt are good sources of probiotics. But this is not always the case.

The bacteria used in the production of yogurt is usually cow bacteria which doesn’t stay in your gut and leaves your body when you excrete.

Many yogourt brands include ‘live and active’ cultures in their ingredient list but this isn’t true all the time.

For yogurt to be classified as probiotic, it must contain clinically tested probiotic strains of live bacteria which are still alive by the expiry date.

Other fermented food items like Sauerkraut and Kombucha are made using live and active bacteria. However, these items usually go through a long shelf life period before they reach consumers and the bacteria will already be dead by then.

If some bacteria do manage to reach your stomach, the acidic environment may prevent them from being fully effective in your body.

For this reason, food items that are considered ‘probiotic’ cannot be trusted to give the same results as probiotic supplements.

2. Probiotics Are Necessary Only If I Am Taking Antibiotics

People who are prescribed antibiotics are usually recommended to pair them with probiotics.

This is because they help prevent conditions like diarrhea, and secondary infections, ease symptoms of irritable bowel syndrome and restore your gut microbiota.

However, the statement that probiotics are only useful if you are taking antibiotics is false.

PANAMA RANKS AS NO.1 RETIREMENT DESTINATION

In accordance with International Living’s 31st Annual Global Retirement Index, Panama has been named the world’s #1 destination for 2022. This Index, produced in accordance with numerous statistics and on-the-ground input from in-country correspondents, ranks and rates the world’s leading retirement locations. The index surveys 10 major categories that include cost of living, retiree benefits, healthcare and more.

Panama snatches the top spot in InternationalLiving.com’s 2022 Annual Global Retirement Index. Panama capped the categories of opportunity, visa/ residency and benefits/ discounts, while scoring at the top amongst the fitting in and healthcare categories.

For such a small country, Panama is truly emerging – this marks the 11th time that Panama has dominated in International Living’s Annual Global Retirement Index, for all the right reasons.

In 2022, Panama ranks as one of the simplest retirement destinations for travel from the United States or Canada. Panama City, the capital of Panama, is the one true First World city in Central America. Easy access to Panama from all over the U.S. increases the flexibility for travel, with several one-way flights available across the U.S., and cities in Canada and Europe. The modernity of Panama is only a short three hours from Miami, and five hours from New York. Not to mention, the currency is also the U.S. dollar!

Jessica Ramesch, the International Living Panama editor reflects on her experience in the country, stating, “as a single woman I feel safe and free to live my life here, whether I’m going out to dinner and Ubering home late at night or driving cross-country to visit friends. Perhaps that’s because this is truly a land of opportunity, home to thousands of hard-working, upwardly mobile locals and immigrants.”

Ramsech continues her advocation for Panama, touching on the accessibility, indulgences, and credibility of the country. She states, “My favorite spot right now is Coronado, a happening beach town just an hour’s drive from the capital. In fact, I like it so much I bought an apartment there, and am packing up to move. Coronado is home to one of Panama’s most active and welcoming expat communities. I’m not even there yet, and I’m already getting invites to potlucks and happy hours.”

No matter where retirees choose to live in Panama, Ramesch says, they’re likely to be only an hour away from hospitals. In Coronado there’s access to the San Fernando facility. It’s a satellite of the San Fernando in Panama City—a JCI-accredited hospital affiliated with the Miami Children’s Hospital, Baptist Health International of Miami, and Tulane University Health Services Center and Hospital Clinic.

“Panamanian doctors make patients feel truly cared for,” says Ramesch. “They don’t rush through appointments, and they’ll often give you their cell phone number so you have direct access to them while you’re going through treatment or recovery.

“In fact, I’d say Panamanians in general are the best part of living in Panama. I have made so many wonderful friends here. People who are welcoming and fun and have a wide range of interests, so the conversation is always engaging.”

According to International Living, one of the most attractive features of living in Panama stems from the Pensionado program suitable for retirees.

Ramesch declares her support for the program, declaring, “the Pensionado program is, hands down, the best retirement visa program in the world. It’s a big reason Panama has taken the number one spot on International Living’s Annual Global Retirement Index so many times.”

Part of the reason for why Panama continues to steal those number one spots is due to the discounts that retirees with this visa have access to. The program makes it exceptionally simple – and inexpensive – to become a permanent legal resident. All of Panama’s legal residents are permitted to 20% off prescription medications if they’re of “pensioner” age. The base age is remarkably low, too – just 55 for women, 60 for men.

The Pensionado advantages expand to other discounts on dental care and transportation. Other leisure’s such as entertainment and hotel stays are available at low-cost rates, too.

Applicants with income of at least $1,000 a month qualify for the Pensionado program, and once you’re approved, you can appreciate all of the reductions.

“If you’d like to apply with your spouse, you can qualify with less than $1,000 each. You can even include dependents if you need to,” says Ramesch. “You just need a pension of at least $1,000 plus $250 for each additional person on your application.

“The $1,000 pension requirement reflects the low cost of living here in Panama. While it’s true that most of the North Americans who’ve chosen to retire here spend upwards of $2,000 a month, there are expats living here on far less.”

There are great areas in Panama that have rent prices as low as $300 to $500 a month, leaving extra money for retirees to spend elsewhere.

For those not 100% ready to make the permanent move just yet, Panama has a variety of alternatives for legal residency.

“The country has just unveiled a new extended-stay visa for digital-nomad types: The Temporary Telework Visa,” Ramesch says. “It allows you to come down and stay here—not for three months or even six, but for nine months. And you can extend for an additional nine months. That’s a generous total of 18 months, and the main requirements are simple: you must have medical insurance and an external source of income (at least $3,000 a month).”

Panama, too, has a “Foreign Professionals” Visa that has been available for over eight years. It allows foreign nationals to work in Panama, with easy requirements; you need a university education and a job arranged in Panama.

Avoid images and incorporate your LinkedIn account into your contact information instead. Tailor each resume you send out to fit the company role you hope to fill.

  1. Write Cover Letters

Personalize your application further by also including a cover letter. There you can share what you can bring to each company and why you’re excited to work with them.

Make it clear that you’ve researched their company and that you align with their values. Be clear and personable, and end with a thank you.

  1. Apply In-Person

If you can, deliver your resume and cover letter in-person. Making personal contact with your future employer makes it far more likely that they’ll hire you.

Hiring is a time-consuming and challenging process – it’s not fun for companies, either! Give relief to the hiring manager by making yourself an easy choice for the position.

  1. Pass the Interview

Interviews don’t have to be stressful. Remember: an interview is your chance to get to know a company just as much as it’s their chance to talk to you. Ask questions, listen, and be genuine.

Working with a company is a partnership, and an interview helps both parties know if it will be a long-lasting and successful one. Do your research and connect genuinely with your interviewers.

  1. Follow Up

After applications and interviews, send follow-up emails. This extra bit of human contact shows companies you care and reminds them of your name. You know that you’d be a fantastic employee – show them that by using your excellent people skills.

Be creative and take every opportunity to show companies that you’re personable, motivated, and genuine. Hopefully, you’ll hear back from several potential employers soon.

At this point, you’ve done everything you can. Take a breather, and rest. Remember that it can take a long time for hiring agents to get back to you, so be patient.

It’s a good idea to repeat the cycle of assessing, researching, and applying at least once while you wait for responses. More experience with the process may have changed your priorities or opened up new questions. Don’t be afraid to branch out and try something you didn’t initially think would work for you.

Medjet COVID-19 Air Medical Transport Benefits

Medjet, the industry leading air medical transport and travel security membership program, announced today that it has expanded its transport benefits for COVID-19. Effective today, Medjet members who fall ill from COVID-19, and become hospitalized while traveling globally, may now be transported to a home hospital of their choice. In October of 2020, Medjet was the first program of its kind to add COVID-19 transport benefits, but previously had limited transport services to travel within the US, Canada, Mexico, Central America and the Caribbean.

“As more and more countries in Europe, Asia, Africa and South America have opened their borders to travelers, we have increased our COVID-19 transport capabilities. Our members’ safety is always our top priority, and we continually evolve our services based on member needs,” said Mike Hallman, CEO of Medjet.

The expansion of global benefits will not increase Medjet membership prices, and benefits are applicable to both existing and new MedjetAssist and MedjetHorizon memberships as of July 12, 2021.

“While COVID is still a top concern for travelers, our own internal polling (in May) also showed that members were still very concerned with all the other accidents and illnesses that kept us busy pre-COVID. I think there’s now a heightened awareness of vulnerability, overall, while away from home. We hope that being able to get members back from more destinations, even for COVID, gives them the confidence to enjoy traveling again,” said Hallman.

Despite the increased cost of providing transport for a COVID-19 positive member, Medjet will continue to absorb 100% of all costs associated with the transport, and maintain its “no cost caps on transports” policy. A regular air ambulance transport can cost up to $30,000 domestically, and up to $180,000 internationally. Medjet memberships start at $99.
